Researched for: Rimonabant evidence and protocol review
Also known as: SR141716, Acomplia
Rimonabant was the first selective CB1 cannabinoid receptor antagonist/inverse agonist introduced into clinical practice for obesity and cardiometabolic disorders, and was effective in mitigating obesity and its complications in trials. However, it was withdrawn from the market (it was never FDA-approved in the US, and was withdrawn in Europe) due to markedly increased rates of CNS-mediated neuropsychiatric side effects - depressed mood, anxiety, irritability, insomnia, and suicidality - which halted further therapeutic development of the entire first-generation CB1-antagonist drug class. This history has directly motivated research into 'second-generation,' peripherally restricted CB1 antagonists (with limited brain penetrance) that aim to replicate rimonabant's metabolic benefits in rodent obesity/diabetes models without the CNS side effects.

Rimonabant was authorized in Europe at 20 mg daily, suspended in 2008, and withdrawn in 2009 after the benefit-risk balance became unfavorable because of psychiatric harm. This historical regimen is retained for database completeness and is not a present-use recommendation.
