Neriifolin is a cardenolide glycoside consisting of digitoxigenin glycosylated at the 3-position with a 6-deoxy-3-O-methyl-alpha-L-glucopyranoside residue. ChEBI records its occurrence in Cerbera and Thevetia species and classifies it as a cardenolide glycoside with toxin, cardiotonic-drug, neuroprotective-agent, and cardioprotective-agent roles. Experimental studies have examined its cardiac electrophysiology, toxicity, and cellular effects; these findings are primarily preclinical or case-associated.

Neriifolin is a toxic cardiac glycoside. In a human induced-pluripotent-stem-cell-derived cardiomyocyte microelectrode-array study, it produced concentration-dependent pro-arrhythmic effects and complete beat arrest at 300 nM. A 2025 LC-MS/MS case-analysis study reported a calculated serum biological half-life of 19.2 hours in the context of Cerbera odollam poisoning; this value should not be generalized as an established pharmacokinetic parameter for other contexts.
