Enlicitide, formerly identified as MK-0616, is an orally bioavailable macrocyclic peptide PCSK9 inhibitor. The marketed U.S. formulation is enlicitide decanoate (Lipfendra), supplied as a 20 mg enlicitide film-coated tablet. It is indicated as an adjunct to diet and exercise to reduce LDL cholesterol in adults with hypercholesterolemia, including heterozygous familial hypercholesterolemia. Enlicitide binds PCSK9 and inhibits its interaction with the LDL receptor, supporting receptor recycling and hepatic LDL-cholesterol clearance.

Clinical development used the code MK-0616 and included enlicitide chloride and enlicitide decanoate forms. Early phase 1 studies evaluated single oral doses of 10, 35, 100, 200, and 300 mg and multiple once-daily oral dosing at 10 or 20 mg under fasting conditions. Phase 3 CORALreef trials evaluated 20 mg once daily. The CORALreef Lipids trial reported a placebo-adjusted LDL-cholesterol difference of approximately -56 percentage points at week 24, while the CORALreef HeFH trial reported approximately -59 percentage points. Cardiovascular-outcome evidence remains dependent on the dedicated outcomes trial and should not be inferred solely from LDL-cholesterol reduction.
